Returning Patients

-Valid ID (Drivers License, Passport, Colorado Tribal I.D, Colorado I.D, Colorado Digital I.D.)

-Valid Medical Card (If digital, MUST be a PDF)

(If we have a photocopy of any documents above you will be asked to send a PDF version)

EPC Patients

-Valid ID (Drivers License, Passport, Colorado Tribal I.D, Colorado I.D, Colorado Digital I.D.)

-Valid Medical Card (If digital, MUST be a PDF)

-Valid Physician Certification (If digital, MUST be PDF)

-Valid and completely filled out UCF (Photos accepted)
